You will not be able to volunteer until you have been cleared through a criminal background check. You can download the Criminal Background Check Form from the 4J School District Webiste: w.4J.lane.edu/files/4J_vol_background.pdf Please fill out the form and turn it in at the office.

Once you pass the background check, we will note it on your account. If you have already been cleared, you do not need to do it again.
